Hipossalivação tardia, fatores associados e impacto na qualidade de vida de sobreviventes do câncer de boca e orofaringe tratados com radioterapia

Resumo

Objetivo: Avaliar a prevalência de hipossalivação tardia em sobreviventes do câncer de boca e orofaringe tratados com radioterapia e possíveis associações com o tipo de tratamento recebido e o tempo decorrido do término do tratamento, bem como avaliar o impacto da salivação na qualidade de vida dos pacientes.

Métodos: Foi mensurado o fluxo salivar estimulado de 50 sobreviventes do carcinoma epidermóide de boca e orofaringe que concluíram a radioterapia há pelo menos 03 meses e foi considerado hipossalivação um fluxo < 0.5 mL/min. Dados demográficos e referentes ao tratamento foram coletados dos prontuários. A qualidade de vida foi avaliada através do questionário da Universidade de Washington.

Resultados: O volume médio do fluxo salivar estimulado foi de 0.43 mL/min (±0.77) e a média de tempo decorrido do término do tratamento foi 36.96 meses (±42.52). Hipossalivação tardia foi observada em 82% da amostra e impactou negativamente na qualidade de vida dos sobreviventes (p=0.042). Houve correlação positiva entre o fluxo salivar estimulado e o tempo decorrido do término da radioterapia (p=0.017), mas não houve associação entre o tempo decorrido e a hipossalivação. Não houve diferença estatística na prevalência de hipossalivação tardia nos pacientes tratados com radioterapia associada à quimioterapia quando comparados àqueles tratados apenas com radioterapia.

Considerações finais: A prevalência de hipossalivação tardia foi alta na amostra estudada e impactou negativamente na qualidade de vida dos sobreviventes. Os resultados apontam para a necessidade de um olhar mais cuidadoso para esses fatores, visando promover uma melhor qualidade de vida para os pacientes a longo prazo.
