Objective: to understand the experiences of mothers of children with orofacial cleft during the search for health care in the context of the Unified Health System. Method: qualitative study, based on the clinical-qualitative method grounded on family resilience from a systemic perspective. Mothers of children with orofacial clefts attended at an institution in southern Brazil participated. Data collection took place in August 2023, through semi-structured interviews. The narratives were submitted to qualitative content analysis. Results: eight mothers participated, with the construction of two categories that emerged from the data: the trajectory in the face of the diagnosis of cleft and the construction of resilience and the challenges and resilience of mothers in the therapeutic itinerary of children with orofacial cleft. Conclusion: the mothers revealed challenges in the therapeutic itinerary of children with orofacial clefts. Family resilience favored coping with adversity, subsidizing health practices more aligned with the principles of the Unified Health System.
