How to quit smoking during pregnancy? - A systematic review

Aims: To describe the main documented care strategies aimed at smoking cessation by pregnant women. Method: This is a systematic review, guided by PRISMA and registered on the PROSPERO platform (ID: CRD42023413818). A high sensitivity search was performed in the Pubmed, Scielo, Lilacs and Medline databases by combining the descriptors “Pregnancy” and “Tobacco Use Cessation”, as well as their entry terms, with the Boolean operators AND or OR. Randomized clinical trials published between 2018 and 2023, in English, that answered the research question were included. The quality analysis of the studies was carried out using the RoB 2 instrument. Results: The initial bibliographic search located 29,360 articles, 8 of which were selected to compose the bibliographic sample, most with low risk of bias. The sample consisted of 3,288 pregnant smokers. Interventions involved: follow-up through counseling sessions, sending messages of encouragement and feedback on infants' exposure to nicotine, financial incentives, treatment with bupropion, use of electronic cigarettes and nicotine patches/inhalers. Of these, only the financial incentive proved to be effective in reducing/cessating smoking. Final considerations: More studies are needed to investigate strategies for smoking cessation during pregnancy, especially considering community-based family approaches.
