Como cessar o tabagismo na gravidez? - Uma revisão sistemática

Objetivo: Descrever quais as principais estratégias assistenciais já documentadas visando a cessação do tabagismo por gestantes. Método: Trata-se de uma revisão sistemática, guiada pelo PRISMA e registrada na plataforma PROSPERO (ID: CRD42023413818). Realizou-se uma busca de alta sensibilidade nas bases de dados Pubmed, Scielo, Lilacs e Medline através da combinação dos descritores “Pregnancy” e “Tobacco Use Cessation”, bem como de seus entry terms, com os operadores booleanos AND ou OR. Foram incluídos ensaios clínicos randomizados publicados entre 2018 e 2023, em inglês, que responderam à pergunta de investigação. A análise de qualidade dos estudos foi realizada através do instrumento RoB 2. Resultados: A pesquisa bibliográfica inicial localizou 29.360 artigos, sendo 8 selecionados para compor a amostra bibliográfica, a maioria com baixo risco de viés. A amostra foi composta por 3.288 gestantes tabagistas. As intervenções envolveram: acompanhamento por sessões de aconselhamento, enviadas mensagens de incentivo e feedback sobre a exposição dos bebês à nicotina, incentivo financeiro, tratamento com bupropiona, uso de cigarros eletrônicos e adesivos/inaladores de nicotina. Dessas, apenas o incentivo financeiro mostrou-se efetivo na redução/cessação do tabagismo. Considerações finais: São necessários mais estudos que investiguem estratégias para a cessação do tabagismo na gestação, em especial considerando abordagens familiares comunitárias.
