Implications for Nursing Technicians in Palliative Care within Intensive Care Units: A Practice Report

Abstract

Objective: to report encounters with Nursing Technicians (NTs) during shifts in an adult Intensive Care Unit (ICU), aiming to provoke reflections on the professional, affective, and ideological implications in palliative care practice. Method: a practice report conducted between June and July 2024 with 42 Nursing Technicians, totaling seven encounters, using the conversation circle modality. Results: the encounters highlighted the participants' implications related to the perceived low effectiveness of care practices, a hospital environment structured by rules and norms, and the NTs' limited knowledge regarding the implementation of palliative care practices. Conclusion: through the collective analysis of their libidinal, ideological, and professional implications in the care environment, participants were able to express experiences and needs, bringing to light unspoken aspects and fostering deeper reflection on palliative care practices.

Carla Andréia Damasceno Fonseca, Universidade Federal Fluminense

Nursing Degree - State University of Rio de Janeiro (UERJ) 1999. Qualification in Medical Surgical Nursing - State University of Rio de Janeiro (UERJ) 1999. Specialist in Oncology - National Cancer Institute (INCA) 2010. Chemical Dependency Counselor - Federal University of Santa Catarina (UFSC) 2011, Specialist in Adult, Pediatric and Neonatal Intensive Care (CEESP) 2020. Master's student at the Fluminense Federal University (UFF) 2023 - Professional Master's in Health Teaching - Training for SUS (MPES). Works as a nurse at Hospital Ferreira Machado (City Hall of Campos dos Goytacazes) being on duty in the Intensive Care Unit and Nursing Coordinator of the Surgical Clinic. Professor in the Technical and Undergraduate course in Nursing at the Fluminense Federal Institute (IFF Campus - Guarus). She has experience in the areas of Intensive Care, palliative care, oncology, hematology, medical clinic, surgical clinic, emergency, trauma, chemical dependency and hospital management and as a teacher in the areas of Nursing Fundamentals, physiology, psychology, elderly health, medical clinic, surgery, intensive care, palliative care, complementary integrative practices and women's health.

Ana Clementina Vieira Almeida, Universidade Federal Fluminense

She holds a degree in Nursing from the University of São Paulo (1968), a master's degree in Nursing from the Federal University of the State of Rio de Janeiro (1996) and a PhD in Nursing from the Federal University of Rio de Janeiro (2001). She worked as a nurse and technical assistant to the director of ERSA 37 at the State Secretariat of São Paulo (SES/SP), advised municipalities on the municipalization and implementation of different health programs of the SES/SP, coordinated the Nursing course at UNIGRANRIO, was Coordinator of the interdisciplinary Program Lapa Saudável and its subprojects, developed activities as an Adjunct Professor of the Nursing Course at UNIGRANRIO, worked as a researcher with the research project "Interdisciplinary Knowledge and Practices in Primary Care". She has experience in the area of ​​​​public health, working mainly on university extension projects in the following themes: primary care; health promotion, citizenship, family health, elderly health; school and adolescent health, humanization, teaching-learning process, interdisciplinarity. Currently participating in a multicenter research project "Professional practices and health training in the SUS: contributions and innovations of Institutional Analysis for Public Health", coordination: Prof. Solange L'Abbate (Unicamp). Participates as a guest professor in the Professional Master's Degree in Health Education at the Fluminense Federal University, in the health training research line.

Janaina Luiza dos Santos, Universidade Federal Fluminense

Post-Doctorate from the Institute of Studies in Public Health at UFRJ (2023/2024). PhD in Health Sciences from USP - Department of Psychiatric Nursing and Human Sciences of the Ribeirão Preto School of Nursing, EERP - (2013). Master in Health Sciences from USP - Department of Psychiatric Nursing and Human Sciences of the Ribeirão Preto School of Nursing EERP - (2010). Graduated in Nursing from the State University of Rio de Janeiro - UERJ (1999). She was a Resident in Nursing, specializing in Clinical Surgery at the Pedro Ernesto University Hospital HUPE / UERJ (1999/2001). She is an Effective Member of the Research Groups: CAESOS - DEPCH - EERP / USP; Research Group Multi-referential Expressions of Care in Health, Society and Nursing - EXPENF Department of Nursing of Rio das Ostras / UFF and Research Group in Spirituality and Health / ISEC-UFRJ. Taught at Estácio de Sá University (2016/2018), Nova Friburgo campus in the Nursing course. Taught as an Exclusive Dedication Professor at the Department of Medicine at UNICESUMAR - PR (2013 to 2015). Worked in extra-hospital emergency at SAMU / RJ and in-hospital emergency at Barra D'Or Hospital in Rio de Janeiro. Currently, she is an Assistant Professor at the Fluminense Federal University, where she teaches classes in the subjects of Nursing in Adult and Elderly Health and in the Elective Subjects Thanatology, Palliative Care in the Plurality of the Human Being and Bioethics in Nursing at the Nursing School of UFF, Rio das Ostras campus. At this same institution, she is the Coordinator of the Academic League LAPICS-UFF, and Coordinator of the Extension Project CINECLUBE da MORTE, where she offers reflective and sincere conversations. She has extensive experience in the area of ​​Nursing, both practical and academic, with an emphasis on Adult Health, working mainly on the following topics: Death and Dying, Palliative Care, GRIEF, Educational Nursing, Emergency and Psychiatric Emergencies, in addition to Integrative and Complementary Practices in Health.

Lucia Cardoso Mourão, Universidade Federal Fluminense

He completed a post-doctorate at the École, Mutations et Apprentissage (EMA) laboratory at the Université de Cergy-Pontoise, France (2014), has a PhD in Public Health from the State University of Campinas (2006), a Master's degree in Nursing from the Federal University of Rio de Janeiro (1993), Specialization in Health Education from the Universidade Federal Fluminense (1991) and Management Development of a Basic Health Unit from the National School of Health Pública-FIOCRUZ (1997); and a degree in Nursing and Midwifery from the Fluminense Federal University (1985). Leader of the Institutional Analysis and Health Training Research Group - UFF/CNPq. Participates in RESEAU International RECHERCHE AVEC which includes members from Brazil, Mexico, France and Canada coordinated by Gilles Monceau (Université de Cergy-Pontoise - France) and Marguerite Souliére (Université de Ottawa- Canada). She is currently an associate professor at the Institute of Public Health, working on subjects in the Undergraduate Course in Medicine and in the Professional Master's Program in Health Teaching, at the Universidade Federal Fluminense. She held the position of Deputy Coordinator and Coordinator of the Professional Master's Program in Health Teaching - UFF.
